Any recommendations for early (40) onset ED.

I am (only?) 40 years old and have intermittent ED. I think my situation is complex / unique, here is some context:

- Have had some symptoms for ~3 years. Coincides with relationship issues of (still) ongoing long-term relationship.
- started / continue practicing edging to prolong sex (unsure if related).
- did do very long road bike rides (5+ hours) for some months, but no known injury, stopped doing this.
- usually DO wake up with strong erection. sometimes get spontaneous erections.
- some good normal sex (33%), some good sex after lots (5-20 min) of manual stimulation (while giving oral or other to woman) (33%), sometimes ED after manual stimulation and no go (33%).
- usually if lots of manual stimulation is needed, I am ready to climax when I get hard (getting hard coincides with being ready to climax here), at which point I can have decent sex by edging, but that is unusual (unhealthy?).
- improved success with Sildenafil and Tadalafil using as needed, but not 100% fix.
- difficult for me in the mornings, usually get excited then goes soft after a few minutes, not really an issue later in the day.
- been to a urologist, took tests and testosterone is normal / high side for my age. The urologist told me my situation is normal for my age group, to space sex out more, and offered no advice beyond being more imaginative towards sex.
- easier (though still could have issues) to have sex ~once a day, possible but much more difficult to achieve 2-3 (woman is very understanding / enjoys other parts of it, but also very sexual and I would like to be consistently capable of 2).
- in great physical shape (daily routine) and mediocre sleep balance (probably 5-6 hours average), Under high relationship stress, minor work/family stress.

I'm looking forward to any thoughts other members may have here, maybe somebody is in a similar situation, and we can talk in more detail about what works and what doesn't. I have a feeling I have some complex mix of phycological and physical issues. Thanks.

A 24-hour refractory period is fairly typical for a 40-year-old man.
Age: 68. Struggled with ED/PE for years.
Used Viagra for 10+ years with mixed success.
In May 2022 started using Trimix with very good results.
Feb 2023 developed PD

Sounds quite healthy man.
65 yrs, had venous leak all my life, sildenafil and other pills don't work anymore properly, using Caverject with pills.

markgreen: In my thirties I discovered that I had to have a woman of my physical type to have 100 percent reliable sex. I'm thin and trim and could have sex with plump women if I enjoyed their company, but I'd have occasional ED. Maybe this is the psychological issue you refer to?

Sogwap describes me at age 40: I would need a ten to do her twice in a day!

I have a unique melatonin regimen for better sleep. PM me if you want details.
Age 78 in 2023. On testosterone replacement due to hypothalamus malfunction. (Attention depressed guys: low testosterone is a cause.) Healthy health nut but ED due to getting old. Like to keep enough cardiovascular ability to thrust for 30 min.

55, married 31 years to an amazing woman, BPH, PD (47deg down to 35), current Xiaflex treatment with Pentoxifylline & Cialis. Penile modeling with Restore X. Occasional ED triggered from pain response during intercourse.
