Are you getting nocturnal erections?

Like @oldbeek asked. It is important that you are getting nocturnal erections. Since there is little blow flow in our penis are bodies are designed to get blood flow there every night multiple times to keep the tissue healthy. That is why a VED is suggested for prostate removal rehab.

I had a prostatectomy in 2016. After the surgery I guess I lost about half inch. I did try a VED but couldn’t get on with it. I had no firm erections or nocturnal erections. Then in 2018 I had to have radiotherapy due to the cancer returning. I then had no erections after that. Until I woke up one morning in 2020 With a raging hard on :D But during the last five years my wife and I reckon the total length lost is around the two inch mark
But it’s good to now have a functional penis which is about 5.5 inch’s on a good day. But it’s enough to keep the wife happy.
I have just started giving the VED another go and so far I have had no problems.

Like others have posted penis health is dependent on Nocturnal erections. These usually happen at night. These start before we are born and happen from that time on. These erections keep the tissue healthy and elastic. If these stop or are reduced greatly atrophy begins. Dose not make any difference the reason. ED, cancer, ect... There are publications on the use of Vacuum Erection Devices (VED) to help with this in the Documents worth reading thread at the top of this page. VED therapy can slow down greatly this atrophy but is a lot of work.

I have a theory that the use of a pump will ultimately cause whithering of the portion of the penis that is not inside the cylinder (causing droopy erections/reduced length etc). Likewise as mentioned here already, if you do not have regular erections that would also cause atrophy.

I talked about it here in a different post and a suggestion to try using a cheap repurposed pill intra-urethrally to get a long erection every night.

AFAIK, even for healthy men nocturnal erections slowly fade away with time.
I've had ED all my life but i did get "regular" nocturnals when i was in my (late)teens. Never had a "raging boner", but decent hard and standing ones were common.
As time went on, i've had less, especially once i started having regular, serious sex life.

Nowadays we usually have sex 2-3 times per week so i practically get no nocturnal erections at all, at least not something i'd call "erection". I do get semi-hard and plump, especially when i wake up in the morning.
All in all, i've never lost size or anything else, so take all that "disuse atrophy" thing with a grain of salt.
