If it's not a venous leak, what else could it be?

Little background on me.
I'm 38 years old and I've suffered from some form of ED or another since my mid-20's. I've been married for 11 years, have three kids and have been with my wife for the last 15 plus years. Up until about a year ago we had great sex. Little, to no issue with losing erections whatsoever. I could generally outlast her on a regular basis. She could get to climax during intercourse at least once, sometimes twice and on a really good day, theee times during one sexual engagement. I could wait until she got off, then essentially and consciously decide, "well, I'm ready now." It was with ease.
Now, the first time I ever lost an erection was when I was 22 and it was with a girl that I was super-crazy excited about. So I chalk that up to performance anxiety. It happened again with another girl the first time at about 24 (me and my future wife had broken up at the time). My wife had our third child back in March of 2017 and since then I've had significant issues with ED to the point where I saw my doctor about it and I'm scheduled to go to him again in about a week.
He prescribed Viagra and I've had limited success with it. I have had very mild high blood pressure (usually around 130-85 and I was ok atenolol (50mg) and later placed on Ibresartan (150mg). Recently my doctor took me completely off any mess because he felt like the beta blockers may have been causing my ED issues. However, after about a week I'm still having the exact same issue. I've done a ton of research and read non-stop about it. Here are my symptoms and what I think it is.
I can almost always get good, hard and normal erections. That's typically not an issue at all. A few times here and there it has been, but for the most part I can get a good erecting. However, like many people that suffer from a venous leak, I lose any erection I have in a matter of seconds if there is no stimulation at all. As in, completely erect to completely limp in a matter of 3-5 seconds.
There are also times when we are having intercourse and the exact same feeling comes over me. Just essentially lose sensation and go limp. Sometimes I can regain erection. Other times I can't. A few times I've been able to reach climax half erect, which is really strange for me as I've never really experienced that.
In general over the past 2-3 months my erections are just not as strong as they used to be. It's become a huge issue and the more I read about it the more I'm convinced I have a venous leak. I'll be bribing this up to my doctor to see what he thinks but I'm already thinking ahead about specialsists and what I can do.
Ultimately, I'm hoping that it's not a venous leak but in the back of my mind I can't help but think that's exactly what it is. All of my symptoms feel in line with a venous leak, but if it's not, what else could it even be? I genuinely do not feel like this is a pshycholigical issue. I feel like it is 100% something physical.
Any help would be tremendously appreciated.
