Question regarding taking sildenafil AND Tadalafil together

Something I've never been able to get a clear answer to on Reddit and other places because people seem to misconstrue the question.

Every online and basic medical resource seems to indicate you shouldn't mix both pde5s.

However they also say that other than duration and onset they work in the same way.

Given this to be the case why should taking 50 mg sildenafil and 10 mg tadalafil be any different to taking 100 sildenafil or 20 mg tadalafil?

Bearing in mind I'm not talking about taking both at maximum doses or taking over what woukd be the maximum dose of one combined, but taking a combination like 25 mg sild and 15 mg tad or 5 mg tad and 50 mg sild.

Is there any reason why it should be more dangerous than taking the full dose of one?

CPPS pudendal neuralgia and muscle tightness in late 2023 making ED worse.
Cialis no longer works. Sildenafil 100mg with diazepam works some of the time.

I've moved on to ICI and no longer take Sildenafil; I still do take a daily 10ng Tadalafil for general health more than ED. But my urologist was a fan of combining the two PDE5's. Tadalafil at a low-dose daily and Sildenafil on demand.
Born 1954. Diabetes, hypertension, atherosclerosis. Sildenafil iffy. Tri-Mix 30/3/20 Pap/Phen/PGE1 godsend pending long-term efficacy. Daily Cialis. Tried LiESWT 4 times, Botox, PT141, Eroxon, QST, DUS, Vertica, Ferticare, cabergoline, psycho+hypnotherapy

I say it over and over on this board, do not take Viagra, it is a bad drug. Take Cialis and Levitra, much less side effects. If I had to do it again I would have gone straight to Trimix.

Back in the day when pills still worked, I quit on Viagra. Found that I had better results and less side effects with Levitra. I did 5mg daily Cialis and a Levitra boost for date night. Did that combo for several years with my urologist’s approval as he wrote the prescriptions.

But in regards to my OP.

Am I right in thinking that there's no safety reason why pde5s can't be combined as long as you are staying within the maximum dose of one when combined correctly?

I can't quite understand the science of why the official line is do not combine but also that they work in the same way.

If I am cleared to take 100g viagra or 20 mg tad what is the difference between that and a combination of 10 mg tad and 50 viagra ( or the levitra equivalent)

If they haven't been studied in a clinical trial using the combination of the two, then the standard line is likely to be don't take them together. They are both vasodilators, so perhaps the worry is the combination in some people, with potential cardiac issues, it might cause a serious cardiac issue.

I'm guessing that the combo use would be considered an "off label" use. In the end, if your doctor signs off on using both, then I would be good to go. He should know all your other medical issues, if any. Personally, in my pre-implant days, I would have never combined them unless my doctor had authorized that usage.
75 YO (1951). Happily married since 1972 (A couple since 1968). ED since age 60. Viagra, then Cialis, Trimix/Quadmix and VED. AMS 700 21cm + 1cm RTE left side, MS pump & 100cc reservoir. Implanted using Infrapubic procedure Jan 21, 2026.
