the field is new and understudied, but there are some promising new about EMS (NOT tens) machines. they can help nerves and muscles directly involed in the mechanism of erections and impulse /desire
they're sold as medical devices if you use ems with anal plugs. you may need a prescription, at least in europe. they differ from tens (they're cutaneous and less potent) shockwaves and radiofrequency(both venogenic approach)
if you're a responder to pt141, it may worth a shot

Any evidence that this will cause structural or other permanent improvements?

started june 17,2025 with anal probe at 17/18hz 220 (neutrotrack pelvitone program11 - relaxing) is undeniable good for erections. how good i cannot say for the moment, they told me i need 4 to 6 months to assess the results

Re: EMS machines for nerves and muscles
Unless I’m being stupid, the only difference between TENS and EMS is the frequency, pulse width, and contraction time.
Having said that, I use a machine branded as a “tens/ems combo”.
I rotate through:
1: glans and perineum
2: glans and lower back
3: perineum and lower back
Using 5hz/150us. I then switch to 50hz/500us on the next day. Contraction time is 3s for both.
I have the intensity as high as I can reasonably tolerate, and I use it for maybe 20 minutes a day.
Got no idea if it helps, but it seems safe, so I’ll persist anyway.

Re: EMS machines for nerves and muscles
I recently bought a Ferticare 2.0. it's intended - and gets good reviews - for men with spinal cord injuries who need stimuli to achieve ejaculation for impregnating a woman. Recent studies, as cited above, show some efficacy for penile rehabilitation. It goes up to an amplitude of 4mm at up to 120Hz.
I find it mildly stimulating but not enough for orgasm. My hope is that through vibration, it will stimulate some nerve regrowth and improve orgasmic response. My uro suggested applying it all along the dorsal nervel down to the pubic bone.
Time will tell.
