Chronic Prostatitis?

My PSA was checked every year as part of my routine physical. It was always below 2. In Feb of 2024 it was 2.5.
In December of 2024 I had my physical a couple months early and my PSA was 12.09! Figuring it might be a lab error it was rechecked 2 weeks later and it was 10.32. Two weeks after that 9.15. Two months later 6.67 (end of Feb 2025).

Since Feb it has settled into the mid 5 range and plateaued there. Last week it was 5.7

I’m 57yo. No prior history of prostatitis. I had no pain. No discomfort. Urinalysis was negative for any infection. My only symptom(?) was that my ejaculate had more volume, was thinner consistency, and shot much further. Not alarming. Not something that sends you to the doctor’s office.

I have a new urologist and he has me on a 10 day course of antibiotics (Bactrim) and anti-inflammatory medication (diclofenac/Voltaren). Then we’ll get another PSA.

He also ordered an ultrasound of my pelvis and that should let him get measurements of my prostate.

His comment about the digital exam was that the prostate was soft, smooth, and small.

I have been reassured that a big, fast swing in PSA values is not cancer. That cancer doesn’t grow/react that quickly.

Does anyone have any experience with a (relatively) symptom free prostatitis? What was the course of action and what happened as a result.

I’d welcome any suggestions.
