Hello everyone,
I've been suffering for four years now with erectile dysfunction due to Hard Flaccid Syndrome, all the hallmark symptoms, which, until recently, I believed was an anomalous occurrence exclusive to myself. It came on during a period of intense, protracted stress due to "idiopathic" symptoms, later identified with a hard-to-diagnose bacterial infection that had become systemic. Infection is now cleared up, but nothing seems to work to relieve the condition (pulsed radial waves; stretches to loosen pelvic floor; nitric oxide boosters; even TRT). I turn forty this month, and so I'm now giving serious consideration to an implant, even though I know many in the HF community are opposed to such an intervention. I just don't know what to do at this point; after four years my symptoms are worse, not better.
Any suggestions anyone could give me to help with this condition would be greatly appreciated. I would especially appreciate it if someone could direct me to anyone who currently has an implant who also had hard flaccid.

P.S. Testosterone replacement, which I tried out of desperation, only exacerbates the symptoms of hard flaccid (creates even greater tightness at base, tighter erections). At least it did for me; I would personally advise against if anyone is considering.

42. Implanted by Dr. Kramer, July 5, 2024, AMS LGX, 21cm cylinders + 2cm RTEs. Idiopathic "hard flaccid" ED following bacterial infection. Tried pulse waves, Cialis, TRT, even spinal injections. Nada.

Out of interest what are these stories about implants and hf and why are people opposed to them?
Have cpps and occasionally the odd hf symptom... But haven't read these stories.

Psych ED since forever less problems being erect alone.
3 kids. Long term partner.
CPPS pudendal neuralgia and muscle tightness in late 2023 making ED worse.
Cialis no longer works. Sildenafil 100mg with diazepam works some of the time.

Hi Never_Enough:
Opposition seems to stem from the contractile nature of hard flaccid, contraction possibly acting as an obstacle to surgical stretching-for-length, hence appropriate sizing.
However, this is speculated to be incorrect on two accounts: 1) that the neuromuscular/skeletal muscle relaxing effect of general anesthesia should temporarily resolve the hard flaccid for implantation; and 2) that the destruction of the corpora via surgical corporotomy eliminates the contraction caused by the bulb of the corpora (the proposed etiology of hard flaccid).
There aren't too many people who have gone this route for hard flaccid. Hard flaccid itself is highly amorphous, with some experiencing contraction and others not so much. There is one person on this board who did receive a penile implant for HF, is happy with the outcome, but as to his previous symptoms I have no clue (whether he had stretch-inhibiting contraction or not). You might try reaching out to him; he goes by the handle "endoftheline." Nice guy; he should respond.

Guys,
I don't very often step in, but implants are not at all a treatment for hard flaccid. HF is caused by tight, inflamed, and spasmed pelvic floor muscles. The treatment is a good physical therapist, plus learning coping skills for stress and holding your tension in your abs/pelvis. Get to a therapist to learn to manage stress. If needed, valium suppositories are helpful. Botox injections sometimes help. It is all about releasing the tight pelvic floor. Once that is cleared up, normal sexual function will return.
There is no easy fix. But the answer is definitely NOT surgery of any kind.

Hi maybe you mean to get better go to pt but i hear is only recover a little but, the pt dont heal it, i will do it everything, leave the masturbation inclusive but a implant is the best solution i think, you dont worry to make a erection, you dont worry for the tense o retract ic muscle, you dont worry of pain or nerves fr low back, you dont worry of ejaculation or lost the erection before erection, i have 28 but if in 10 years maximum im not recovery, i do it the implant first i go to recolect money, sorry but feeling better and later bad dont look for that and feeling bad with my size and all the girls say she wants a big cock so inclusive i leave talk with girls for this condition and all people think like i dont like, i like girls only have this shit for years

I must say I hate to disagree and I respect your experience in ED but implants absolutely are an option for this condition. In fact long term I think they are the only option for some of us. Im not sure if you have PN or PFD/HF but its rarely something that 'clears up'- and I dongt mean that sarcastically, I genuinely mean its very often a lifelong condition.
HF and Pelvic Dysfunction/Pudendal Neuralgia is a very complex, difficult and desperate condition that doesn't always respond well to any of the treatments listed there. Stress management is good, but you need to get a hold on the perpetual pain first and that tends to be cyclical. Valium suppositories might help some but they cause ED themselves- as do pretty much ALL the muscle or nerve relieving medications at varying levels. Botox helped me a little but caused ED as a result of the muscles being unresponsive.
HF is also caused by pudendal nerve damage, which isnt always easily reversed.
Pde5s help somewhat but arent great as they wont get through the issues caused by muscle or nerve dysfunction, nor the stress.
Injections are hard with this condition because the penis can be retracted and rubbery, plus Aprostadil makes pain worse.
I live a difficult lifestyle in constant pain and its likely that the only way to get even some relief will be high doses of nerve medication. There is no ED treatment thats getting round the ED caused by these medications especially since some of them contraindicate injections.
The only good option to the ED caused by moderate/severe PFD and HF are implants- and its something I have discussed with Dr David Ralph.
There was someone on here I spoke with whos implant actually massively helped their PFD as a result of no longer putting so much strain on the BC and IC muscles and there was at least one person on Reddit whos HF was effectively cured by an Implant, so i dont think we should dissuade people, as If I felt implants werent an option I would have probably been in a very dark place.
