I'd never heard of those examinations, how are they done and how reliable are they? And what is this toxic neuropathy you mentioned?

It is very brave from you to share your issues and seek information on ways to help you. Absolutely, an experienced urologist will be able to help you very much.
I just got an implant a week ago. I am recovering very well and smoothly. Clearly, the recovery depends very much on our health condition. I am 41 year old. I don't drink. I don't smoke. I am not overweight. I do sports. Most of people would say I am in my early 30s. Despite all this, I have been experiencing progressive ED problems in the last 10 years or so. So don't blame yourself! You are very brave to reach out for help.
I agree with you on one point. I also thought that the operation would be very invasive and extremely painful. A week later, I can share with you that it was much less painful than I could ever have thought by watching the medical movies on YouTube. Performed by an experienced urologist, you will see that, regardless of the high complexity of the operation, the operation is not as terrible as it may sound. Implants are very reliable nowadays. However, the urologist experience as well as your efforts to strictly follow the recommendations for the aftercare will define much of the outcome. I learned this in this discussion board.
However, you may not need to get an implant to be able to regain your erection. Let the doctors study your case in detail and recommend you the best course of action.
Last but not least, I fully trust that you will be able to make your dreams come true. Don't give up. The most important step that you could have taken, you took - you are seeking help.
I am cheering for you to find the best solution for the ED problem. Don't be afraid or give up! Keep the head up and you find what you are seeking.

Well, unfortunately all the available examinations to seek problems with nerves and nerve conduction (especially in the complex pelvic area) are not too reliable.
What I learned now is that only specific examinations with MR scans can possibly display the nerves. The examinations of measuring the evoked potential (nerve conduction velocity) of e.g. the nervus pudendus or the urodynamics examination, which I did both, they only show that there's a potential delay or malfunction but it can't show really clear results or indicate what might be the cause. But it's not examinations which are super advanced or anything, these 2, it's more like interpreting superficially triggered stimuli.
So now I hope to get more clarity with different MRI examinations.

Thank you for sharing that study, it's always interesting to explore the latest findings in this area related to ED.

I'm really glad that it went so well with your implant surgery. That's great news and really encouraging that it was smoother than expected. Takes me some fear, in case I really have to get the implant soon.

I fully understand your feelings. I was also very scared in the day of the operation. This is the reason why I decided to post my experiences. The good thing is that there is help today. I am very happy that you are feeling well and more optimistic. If an implant is the solution for you, don't be afraid! With an experienced doctor, you will get great results and you will be able to enjoy life even better than before. It is very impressive how my mind has changed since then. Two days ago, I received the green light for sex. Well, we are in lockdown in the UK. I remembered that I still had a fleshlight which I was able to have some fun with quite time ago. I must say it was great to be able to play with this again. My penis is a bit girthy and the fleshlight so tight. In spite of this, no problem. Just the glans were a bit sensitive but it was not pain :D , a different type of sensation. I hope that the lockdown ends soon :lol:
The implant has not changed the penis sensitivity, actually it feels better now because it is hard as a bone. Definitely, I am enjoying and 1000% satisfied.
I feel that, more than erections, I am regaining my self-esteem and dignity back.

Until I was 21 everything was great for me. I had the best erections and super high libido. But then suddenly, strong erectyle disfunction hit me unexpectedly.
It came all of a sudden 3.5 years ago, and was at a time of a lot of alcohol consumption during approx. 2 weeks in my exchange study semester,
so that's probably what caused it (since it's the only possible explanation for me). I didn't think it was permanent and that it even gets worse and worse, I just had no clue about anything like this.
It started off already pretty severe and made me very insecure. Finally, one more party vacation with my friends ruined everything. I highly regret this decison, to have taken that vacation, maybe before my nerves could've still being able to recover.
Then however, I was at the point, where I never got any spontaneous erections, neither during the day, night or morning (maybe slightly occasionaly in the morning) – so I'd say the ED got into a really advanced state.
The problem was I never really knew what it was. I am not an alcoholic. I just drank quite a lot during these 2 occasions. Otherwise I live a quite healthy life and do a lot of sports. I was also smoking during my exchange semester though, but otherwise just socially at parties.

2 years ago, I went to see a urologist in my home country in Europe, but he couldn't give me a satisfactory diagnosis in the end,
and just blamed it to be a psychological cause, when it is clearly not – since it's a condition that's always there, even when masturabating.
He didn't recommend me to do a doppler ultrasound or the like, for which I asked for to find out more.
Ever since, I tried all the different available medications and dosages (levitra, cialis, viagra...), and found that only the strongest dosage of sildenafil (100mg) could mostly help me to achieve intercourse, but even then it wasn't quite satisfactory and also expensive. I could also only come once, every time I took one 100mg pill, which wasn't satisfactory either.

My last and most recent attempt to make my ED better, was doing the low intesity shockwave therapy. I did 5 treatments, with a lot of hope. However, it only made things worse (probably damaged more nerves) and I should have done more research before going into it.
Though, now I found out with quite certainty, that my ED occured because of a autonomous neuropathy, caused by alcohol abuse within a short amount of time. Even though I told my urologist the same possible reason (which I had thought could be the cause), he said that this couldn't be plausible, in such a short amount of time. I now believe though that my nervous system was just too weak and that also bad nutrition lead to the nerve damage.
Now I am pretty sure about the cause. However, now I'll do all available tests to get a medical diagnosis, which still can be quite hard to get since it's tough to figure out nerve damage. My current state is that even with 100mg of viagra, I have a quite limp penis, despite the highest possible arousal and currently I don't feel like I am able have sex anymore.

Finally my questions are:
Do you think I am on the right way, and did anyone experience a similar history with his ED?
Also, the only solution (last resort) for me right now (even though I fear it a lot), would be to get an implant? Would you recommend me getting an implant soon, or do you guys think that there's still other things to try first and that I should wait more?
I also heard of nerve translpantation, however I have high doubt that it could work in my case (they do it mainly after prostatectomies), does anyone know more regarding this? Or do you think it could be good enough in the near future?
Should I proceed thinking about an implant, since my condition is really advanced, or wait? I mean I would do everything to regain spontaneous erections, but how possible is it in my case that I could get them back?

Maybe some of you can help me on my way to a rather normal quality of life and out of deep depression, thanks a lot!
