Orgasm after prostatectomy... When?

A very new area of interest is penile physical therapy. From exercises to stretching to vibrational therapy, it's all new and still being studied. This is where we can share techniques, exercises and successes.

Orgasm after prostatectomy... When?

Postby Holzwurst » Mon Apr 17, 2023 7:29 pm

Greetings.. I just found this website and it looks like a good place for my question. I am 3 weeks into recovery following RP. I've been taking Cialis for 2 weeks and also pumping as part of my recovery. My question is specifically about orgasm. While pumping and massaging to stimulate blood flow, I already have a pretty good urge to climax. I haven't allowed it yet because I'm worried it might be too early for some reason. I haven't found much information online, and the little I found suggested waiting at least three or four weeks to have any sexual activity, and another suggestion said to wait 2 months. Another opinion I read seemed to indicate that there's no time limit at all. Anyone have any experience with this? How soon after surgery can we safely rip one out?

I'm not a medical professional of any kind. Anything I post is based on my own experiences at best and hallucinations and/or delusions at worst. Internet forum advice is likely worth what you paid for it, or worse. Always ask your doctor and follow his/her advice.

That said, if I was healed up and the urethra was considered healed, and I was close to the big "O", I think I'd have to let her rip. However it might actually hurt. Beware that there is a strong likelihood that at this point if you do orgasm you will ejaculate a substantial amount of urine. Void completely beforehand but still expect some water works. It will get better with practice.

What did your doctor say when you asked him/her? By this point any humility you had should have been destroyed so asking your doctor such a question shouldn't be an issue.

The day the catheter came out (one week after RP), I asked about orgasm and my doctor said whenever you feel you can. The next day I didn't think I was ready for sex, but I did masturbate to orgasm (granted, with a soft penis). No pain, felt different, but good.

I think I was about 3- or 4-weeks post-surgery for me (nerve sparing robotic prostatectomy) (I had to have my catheter put back in thus the duration). It has been 11 years. Anyway, yes it was a limp wet (urine) noodle with no erection sensation but there was that tingle at the head of the penis. The surgeon gave me the go ahead with some Viagra which I never responded to. Yes, I had an orgasm, it hurt, and it felt good at the same time and yes it felt different.

I had my RALP in early February and didn't do anything, and I mean anything other than urinating for 6 weeks. And then I started up with a VED and generic Viagra (Dr. prescription for each) and didn't attempt masturbating for another three weeks, with very limited success. Finally last week I used one of the rings that came with the VED, feeling my urethra must be healed completely by now. I masturbated with a fully engorged penis and I have to admit it was a good orgasm. No ejaculation obviously, but a much better feeling than I would have guessed. My penis is smaller than it used to be, attributed to the surgery, but it is looking more normal each time I use the VED.

I waited about 6 weeks. I have no EF at all.
Unfortunately, I've also lost most sensation that allows orgasm. I tried a few times pumping up and using a constriction ring... close but no joy. I finally achieved orgasm while flaccid using a vibrator... supposedly vibrator use might help to restore some of the sensitivity that I've lost... time will tell.

Good luck to you.
Age 66. RALP Jan-2023. Non-nerve sparing.
VED exercise most days, Trimix T-106 (30-1-25), 25 units.
Bending trauma around age 50, resulting in mild Peyronies.

You are only a couple months post-op. Be a patient patient. Nerves can regenerate, albeit slowly. It can take months or even years. And besides that, you can develop new neural pathways through which pleasure can be achieved.

I expect that I will be able to orgasm by penile stimulation. My concern is that my girlfriend (GF) and I started exploring prostate stimulation and prostate orgasm (just a few times so far and one - maybe - success at p-gasm). I was wondering if a p-gasm will be totally impossible if I get a prostatectomy for my cancer. Right now the medical evidence I have is equivocal over which treatment pathway to follow, radiation (which pretty much rules out a future prostatectomy) or prostatectomy with a chance of a little bit of radiation to follow up for any cancer that travelled outside the prostate.

Sorry for your PCa situation, but save the prostate if possible (i.e. if you have a less-aggressive cancer). Without a prostate it is highly unlikely that you will have any enjoyable stimulation from where the prostate used to be. And of course, you will no longer be able to ejaculate. Orgasms are still possible, and quite enjoyable, but a bit different from the normal male orgasm/ejaculation. I'm currently only able to achieve an orgasm while flaccid, using a vibrator (I guess that more aggressive, deep stimulation is required). I'm hoping that my penile sensation will return enough that someday I will be able to have an orgasm while erect (thanks to injections), and be able to orgasm along with my wife during PIV sex.

I just got results from a PSMA-PET scan. The cancer is confined to the prostate gland and seminal vesicles with some nerve involvement. Nothing outsitde the prostate is apparant. But the cancer is fairly extensive on both sides and the apex of the gland.

Right now, it (in my uneducated view) looks as if 100% cure might be possible with prostatectomy. It would be nice to be relieved of having this hanging over my head for the next 25 years (if I might live that long). On the other hand, the concerns you mention are dear to me, also.

Thank you, still_crazy

LostSheep: glad to hear the cancer is confined to the prostate.
I'm sure you will find that the RALP surgery is a much easier recovery than an implant. Some people do have problems with incontinence for several months, but I was quite lucky in that regard. I still have some minor stress incontinence from time to time (5 months out), but it's not an issue for the most part.

HolzWurst: Any joy on that orgasm? How are you doing?
