72yo on this site for years but just made it official. Tried all available. Viagra, Trimix and a pump. Had reactions with meds so I was implanted with a coloplast titan in August 2024. Generally happy with it but…

Don’t think it is a problem with the surgeon. Just not exactly what I expected. I wasn’t totally prepared for the outcome. It’s definitely better than nothing.

Welcome brother. Just curious what it is that you are not happy with?
Possible issues that come to mind below. Is it any of those?

Size loss/possibly undersized
Unnatural appearance
Curvature
"Floppy" glans/soft glans/no glans engorgement
Overall lack of engorgement
Numbness/loss of sensation
Feels foreign, not like part of your body
Unable to orgasm/delayed ejaculation
Ongoing pain
Urination problems

In some cases there can be "self help" ideas that might improve things.

You hit on a lot of issues. I wasn’t prepared to deal with these.

I had not heard terms like Floppy head or points that are under the head. And dog ears. When I deflate it. It looks like a pancake. Also in the literature it says you won’t be able to tell you got it. I feel tubes and the pump stickers out on the bottom of my scrotum.
I have been thinking of getting scrotox to loosen my scrotum. After the surgery it has been so tight.
I’m generally happy but I was in aware of all the things. It could be worse. It is useable and I do get an orgasm.

Hi Kyguy47,
If you have both floppy glans and size loss it can be potentially a sign of undersizing.
But another reason can be a lack of cycling. Initially I was shorter and thought my implant tips were not far enough into the glans, and the head seemed floppy. But I started cycling quite aggressively and it didn't take too long for the tips to settle much further into the glans, and size returned. Now they are about halfway in when I'm flaccid and well over 3/4 in there when I'm fully pumped up. So my erect penis is very stable during vigorous sex. The glans is well supported and doesn't move around much. If you haven't done much cycling it's never too late and might be worth a shot. Your Titan is more rigid than my LGX but they all have some elasticity.

Beauty is often in the eye of the beholder. With the pancake look, do you squeeze the residual saline from your penis after you deflate? I know a lot of the Titan guys complain that it looks less natural when flaccid, but say that squeezing it helps. The dog ears are another known issue but again a lot of guys say that with cycling they become less prominent.

Is your curvature significant, or something you can live with? Did you have curvature/Peyronie's Disease, or fibrosis, before the implant?
I had fibrosis/scar tissue removed during my procedure and over time I've developed a noticeable curve. I did have some curving pre op but now its more exaggerated. I'm doing long term physio/bending exercises to correct it, but it isn't a showstopper and actually gives me an advantage during PIV sex by "faking" extra girth. Cosmetically it would be nice if it was straighter but I'm now used to it and not overly worried about it.

I can also feel the tubing at the base of my shaft, which seems fairly common. Mine aren't really visible unless I'm 100% erect, and even then it's not that obvious. Again I feel that ongoing cycling has helped minimize their appearance.

My pump also sits low but I have a fairly ample sized scrotum, so it lives there comfortably. I actually had Botox injected into my bladder wall to correct persistent leakage after my prostatectomy. That has worked well and helped relax my rigid/stiff bladder. So it might be worth considering the Scrotox procedure. One thing with Botox though it usually isn't considered a permanent solution, and may need re-doing eventually. Although so far mine seems to be holding up after nearly two years.

Any foreign feeling for me finally disappeared once I was able to have completely pain free sex. Now I don't notice my implant at all unless I'm actually using it. Again I think ongoing aggressive cycling has stretched everything to the point where it has become comfortable.

Yes the main thing is that it works and you can still orgasm, same here. I regained my pre ED erect size, maybe even slightly more, much larger flaccid, and now orgasm 100% of the time from sex. The few negatives I can live with for that.
