Re sleep disruption: Try low-dose melatonin. An MIT study found that 0.25-0.33mg is correct, depending on your weight. I cut a 1mg tablet into four pieces, taking one at bedtime and another when I awaken two hours later, another if I awaken after another two hours, etc. I usually use 3-4 pieces per night, simply putting them under my tongue - don't get up for a glass of water.

The doses on all containers are much too high and overload your receptors. If you've been taking 1-20mg at a time, like the directions say, go off entirely for ten days to reset receptors.
Age 81 in 2026. On testosterone replacement due to hypothalamus malfunction. (Attention depressed guys: low testosterone is a cause.) Healthy health nut but ED due to getting old. Like to keep enough cardiovascular ability to thrust for 25 min.

Similar to what others have reported nerve sparing surgery did not work for me.
I had Gleason Grade 7 Prostate Cancer, and in 2022 had a radical robotic prostatectomy with 100% bilateral nerve sparing.
Thankfully I've been cancer free since. All PSA checks to date have been zero/undetectable for PC. Grateful that I'm still here, which was the objective of the surgery. My wife and family are also very happy about that. But the side effects of the procedure have been diabolical and soul destroying.
I suffered profound ED for 3 years before recently taking the final plunge and getting an implant after all conservative remedies failed.
I had no erectile function whatsoever after my RRP and never regained any of it, in addition to significant urinary incontinence for 18 months or more.
My belief is that both ED and ongoing incontinence are extremely common post RRP, even with 100% nerve sparing. One explanation that I''m aware of is that even when the visible nerve bundles are spared, there are many invisible microscopic nerves that are not. And all the nerves go into a sort of "hibernation" due to the trauma from the procedure, and frequently never recover. I asked my surgeon if he ever had a patient regain erectile function after 3 years. He said that he only ever had one, who actually started having good erections after 4 years. Apparently he was much younger than me, and I suspect that age may also be a factor.
71, married, Sydney Oz. PC/nerve sparing RRP Mar 2022 caused 100% total ED. Tried pills, Trimix inj, focal shockwave, VED. All failed. Implanted Mar 6 2025 AMS 700 LGX 21cm x 12mm, no RTEs, MS pump, Penoscrotal. Got back to 6.5" BPEL @ 9m. Loving it.

Think of the cohort of men you are asking.

You are asking this question to an erectile dysfunction web forum. OF COURSE it didn’t work. The guys that regained their sexual function and aren’t having problems with with erections are not here.

My best friend growing up had robotic radical prostatectomy with a nerve sparing technique Christmas week of 2024. We talked a couple months later and he was back to normal. Wasn’t having any problems with impotence or incontinence.

I’m not sure what the percentage of success is but this isn’t an accurate place to ask.

August will be my three year mark since I had my prostate removed. It would appear by now that none of the nerves survived the operation. I am still unable to have a natural erection. My TriMix injections work great! I am still able to enjoy getting hard if only by RX. I gave up on my vacuum device. I no longer take the "little blue pill". I have regained some of the length lost due to surgery. Not much, but enough to say my flaccid penis no longer looks like a little boys penis.It also appears that my erect length is greater too. And three years later I am able to enjoy what remains as my sexual being, having very nice orgasms and although "dry", I still get the pleasure of climax.
Age 71. RP March 2023. No longer use Sildenafil. Tri-Mix 20/30/2 @ 30 units. No longer use VED with Vacurect. Penile Rehab Program. No natural erections yet. Solo sex only. Happily married for 41 years.

I know next to nothing about prostate surgery so forgive my ignorance. But I do know first-hand about anorgasmia. My nerves are slowly atrophying and my penile sensation is becoming less and less. Are your sensations that lead to orgasm the same as before? Are the nerves along your shaft (the dorsal branch of the pudendal nerve) as good as ever?
