Had my surgery on April 17/14 and at my 6 week visit with the surgeon requested the same from my surgeon - just received it this week in the mail. The title on it is "Operative Report"

Dr. Steele is my urologist in Kingston, he's amazing; did my slling but isn't doing the implant. I am also in Ontario, Canada. My urologist in Kingston gave me a referal to surgeon in London, Ontario. I met with him on Oct 28, 2013 surgery is scheduled for April 17, 2014. From what my urologist said there are 2 places in Ontario where inflatable implants are covered by OHIP (London & Ottawa...
