It usually takes a very firm squeeze to get the pump started. The doc doesn't release anything, you just have to squeeze it hard that first time.

I gotta agree I have not seen this issue on franktalk before though I've read about the possibility of it in the various medical papers. Please keep us posted with updates. You're very fortunate to be in the hands of perhaps the #1 surgeon in the world for this issue.
