If your doctor allows - and make sure you check so you don't worry about infection - get into a tub of hot water, as hot as you can handle. This will loosen the scrotal skin, thin it out some, and loosen all the stuff inside, as well. Finding the deflate button and everything else becomes much easie...

If I have a valve failure, my method is to try to inflate with a pump or two and the press hard on the deflate button. That frees the valve for me. I can then inflate or deflate as needed. While the David Archy 2-pouch underwear are the most comfortable for me, they do put the penis out in front.
