The tip of the implant has the suture through it wth both ends exiting the glands once the dr pulls the tip of the cyclinder in to place he then cuts one side and pills the whole suture out nothing remains

That is how I saw it. I might be wrong.

The thread pulling (guiding) the tip of the cylinder into place at the glans is called a "traction suture".

The proceedure is explained on page 20 of the "AMS 700 Operating Room Manual" available here in pdf form file :
https://www.google.com/search?q=ams+700 ... NIr8nFF0lw

Viewing this might shed some light on the situation.

As I understand it, the thread extends through a small hole in the hard nylon tip of the cylinder; like stringing a bead. Like a "U;", I don't believe it's physically fastened "to" the hard tip itself.

Both ends of the thread are "threaded" through the needle.

There are now two strands of guiding thread on one side of the needle and two strands of "trailing" thread on the other side of the eye of the needle.

These trailing strands get folded over to "double over" with the "guiding strands".

Initially 4 strands of thread get pulled through the glans from the corpora.

Once the needle is pushed through the glans and pulled out the other side, the set of 4 treads is "un- doubled", the needle is"un-threaded" [removed]; once again leaving just two threads leading to the hard tip.

The excess of one of the two strands gets trimmed off near the glans.

Then the short [trimmed] strand is pulled back through the needle hole in the glans, "through the hole in the hard tip [un-threading the bead] ; until the short "trimmed off end exits the glans through the original needle hole, while being pulled out by the longer end.

I don't believe any thread is actually left inside the penis, but I could be wrong.

Here is a good video of how they utilize the needle to pull the tips into place through the glans.
