AMS 700 information

How many of you that got a AMS 700 implant did NOT receive a card with the label for the implant and extenders on it??? I am told I got a AMS 700 lgx. Been trying since day one to get written information on what was implanted. Today, I called the hospital administration today to get the information. They found the LOT# and the catalog # and told me to call the AMS rep. to get the info I wanted. The AMS rep several weeks ago said she would get me the info. Today I got a sales brochure from AMS. I want this info in case I have a problem in the future.

Hello oldbeek, you have made a good point. My doctor just told me he would use a Coloplast. I had requested my surgical records and it was in the records that had the information as to size, RTE, ID numbers like a serial number, date of manufacture etc. It's probably something all of us IPP patients should know. It seems that all the guys know their size, but maybe not the serial numbers. VegasED

Request a copy of the operation report from the hospital.

Request a copy of the surgery report, it will detail exactly what the surgeon did along with the device implanted as well as extenders. My surgeon gave me the report at my one month follow up.

I was given all the paperwork after my implant surgery. It had all the "peal-off stickers" from my implant placed in the appropriate boxes. These showed the Serial #; size; the RTE info, etc. At my followup appointment with my surgeon, I specifically recall asking him if there was anything else I needed to add and send back or fax to AMS. He said all that was taken care of immediately after my surgery. The copies I was given are for my own information. (Now, if only I knew what I did with them!)

In with my copies was another sample of the pump bulb. That's on my key ring now!

I pressed my surgeon for the info as it was not on the cards in my packet. He advised the stickers were no longer put on the cards. I continued to press for the information and he contacted his AMS rep that provided the device and had the records e-mailed to me. Maybe you could ask your surgeon to contact the rep who was at your surgery and have the info sent to you. There were 2 sheets with my sticker info from the hospital and the stickers for every component of my device.

Guy's I was implanted on May 21, 2018 and was provided a card with all the information about my AMS 700 LGX, so I don't understand the Dr's statement that the cards are no longer provided.

I received my packet with my cards they just did not have the stickers with my device info on the cards. Doc said they do not put the stickers on the cards now.

I read another post about the life time warranty not being honored. Something about the court case saying just because they use that in advertising it is not a real warranty. May be the two have something to do with each other..

The hospital is supposed to send AMS the info and patient ID of who it was implanted in. I contacted Jami from AMS again yesterday and vented my frustration with the run around. In 4 hrs I received an e mail with an attachment showing all the details I have ben requesting. My Dr also said the day after surgery that they don't give out the cards anymore. Jami said so do, some don't. I suggested strongly to make it a policy to hand out the cards with the info. Thanks for the replies.
