Dear Friend,
Although fibrosis is a possibility, but most likely it was just a singular event. After countless of successful shots, I just missed the other day, probably went to close to the urethra.
I encourage you to consider buying an Autoject El autoinjector. It makes the shot virtually painless and gives very predictable results.
All the best!

Make sure you rotate injection sites..at least THREE on each side. You might want to change elevation (9vs 10vs 11:00 etc) as well.

I went through a spell of bending needles almost every time I injected. I did straighten them usually with no problem.
What I did to combat the needle bending was to give the needle a quick thrust once through the skin.
I even use 29 gauge, 1/2" ers.

Inject testosterone weekly.
Implant on 1/22/19 by Dr Avila.

Gleason 6 prostate cancer. Monitoring it for now.
Update: On my last biopsies the cancer wasn't found.

Absolutely nothing to worry about. Forget about that session and carry on. I'm being serious -- not sarcastic. All of us have bloopers every now and then, and there are never any reports of permanent damage.

Keep injecting a long as it works.
I've tried everything -- pills, injections pumps -- and now I am at the final frontier.
