Several years ago, I tried jelqing. Although there was no pain or overtly visible sign of damage, I immediately noticed a stark decrease in erection quality. Whenever I tried to masturbate, it would not get fully rigid, and tumescence was easily lost.
Since then, I've spoken to a few specialists about the problem. Two of those urologists administered cavernosogram tests. According to the doctors, one of whom is likely to be the most reputable and experienced in my area, the results showed good blood flow and no sign of scar tissue/fibrosis. Testosterone and free testosterone levels were also normal. The diagnosis was psychogenic ED, and they prescribed PDE5 inhibitors. One doctor even suggested seeing a sex therapist.

However, the condition has not improved. I am still able to masturbate with some effort, but erections are flimsy/soft and immediately start going flaccid when I stop stimulating with my hand. I find that the PDE5 inhibitors don't make much of a difference.
Since I still don't really know the cause of my problem, it's difficult to devise any kind of plan to potentially improve it. And the abundance of advice can be confusing.
Some people suggest abstaining from masturbation and sex altogether, just giving it time to heal, waiting to see if it improves by itself. Others say "use it or lose it", and suggest VED pumping, traction devices, edging, etc.

Some people say to take nightly Cialis or Viagra (which is more effective?) and a long list of supplements like Citrulline, Pycnogenol, Icariin, Ginger, Ginseng, Acetyl-L-Carnitine, Tongkat Ali, etc. But it isn't clear to me which supplements are most effective and necessary, which are redundant, in what doses they should be taken, or if there are any complications from stacking.
I'm also considering the possibility that I might benefit from more cardio exercise and kegels.

What are my best options for improving erectile quality? And is it likely that I didn't suffer any major, permanent damage if multiple cavernosogram tests came out fine, or should I keep looking for more opinions?

I believe the best thing to do is to not dwell on it as much as you are doing. The drs with the tests have determined there is a good blood flow and accept it. You could need more stimulation from an outside source or a sex toy. I would venture to say that you should get a VED a cheapie from an adult shop and go pumping. You should be able to discover that you actually can keep it up and at the same time you can play with your dick.

Seems overall you are "tight" in thoughts and there really isn't anything wrong with your dick. You should be relieved. Have you tried going for a massage?????

I would like to find that diagnosis reassuring, but it simply doesn't reflect my experience. The difference in function between now and a few years ago is night and day. It used to get hard as a rock without even touching it. Now I need to force it into working with my hand, but even then, it doesn't get rigid, and it doesn't stay up. It always feels soft, and it feels like it's gotten a bit thinner too.
My guess is that I might have fibrosis, since the tissue feels kind of different. It has an almost rubbery texture in some sections, especially closer to the base.

Believe me a injury from jelqing its a serious problem, not psychological. There are many people here injury for those exercises.(for example tryfied, tgruen and others) The problem with jelq is it provokes more collagen than normal in the erectil tissue, which prevents adequate stretching of the erectile tissue. All the people injury from jelqing have scar tissue and also venous leak and this from the glans many times too. Do you have soft glans? Read the tgruen's story he did dopplers cavernosograms and they didnt show his problem until a doctor found it, he said dopplers arent good to find venous leaks and many times cavernosograms fail too. If you watching porn cant maintain the erection it isnt normal. Doctors dont know a shit when it is an injury from jelqing

Dear Friend,

I don't know much about that process, but can't it cause some sort of a nerve damage that would decrease sensation and or interfere with the blood flow. There is no way to show nerve damage on any imaging, but its consequences can be severe.

I don't usually step in to offer ideas, but please consider that you might be experiencing a common, but usually missed, condition called Chronic Pelvic Pain Syndrome - which isn't really a great name. It's also called "hard-flaccid", pelvic floor dysfunction, among a number of other things. All the symptoms you describe sound like that. It is easily treated with physical therapy from a PT who specializes in it (most PT's do not treat this condition). It is exacerbated by stress and anxiety, so keep those in check as well.

Have you tried injections? That would be my top choice and it would tell you whether you have organic vs other issues.
I migrated to injections when pills failed after 15 years of relative success. It was a game changer for me.

I know exactly your feeling. I injured my dick almost 8 years ago now. My initial injury result was a dick similar to what you describe. Mine is certainly thinner, but is approximately the same length. One thing I noticed is it doesn't fill up nearly as much when making a bowel movement now.

In my case, I took Cialis for 2 years from age 21 to 23. This made things tolerable, although my erection angle actually became higher and the erection was not quite as thick, particularly near the base. Also, my flaccid penis was not as "spongy" as before. I was able to function well after that with no medication for 4 years until April when things went downhill after doing the penile stretch test to myself. Now, it has very little sensation, so I am convinced I hurt a nerve, although nothing showed on my 2 MRIs and 2 Doppler ultrasounds.

Over the past 8 years, I have seen 10 urologists and none see anything wrong, but I know for sure things are not the same. It's insane that this is possible.

If I were you, I would first be very gentle when handling the penis. Never stretch it again to avoid hurting it more. Also, I would try Cialis daily for a few months and see what kind of results you get. If this fails, some suggested injections I see. However, if you are young, I think you know how this will end. I would at least send an e-mail to one of the experienced implant surgeons or even do a consultation. I met in person with Andrew Kramer, and he will be my surgeon when I'm ready for an implant.

I actually am doing pelvic floor physical therapy right now. I have my next session in 10 minutes. It involves relaxing the muscles near the anus and scrotum. This, in turn, could loosen things up allowing more blood to flow and perhaps nerves to be free.
