ED, Venous leak? Help!

Hello,

I've been experiencing ED since January 2016. I can get an erection with stimulation, but I get soft within 15 seconds after ceasing stimulation. I also have a lessened sensitivity in my shaft, but this seems to have been improving over the past year.

I've seen multiple urologists, and even a few andrologists. First andrologist said my doppler ultrasound results weren't indicative of a leak, but I had my doubts due to my symptoms. Last month, I saw the head andrologist at Mayo Clinic, and he assured me that this is definitely not a leak. He performed another doppler to put my mind at ease. At my first doppler, I had priaprasm, so he only injected the minimum dosage (.05 mcg) for this one. My peak systolic velocities were above normal, and my end diastolic velocities were slightly elevated (I think 2.85 and 4.3 cm/s). He says this was expected since a full erection wasn't induced. At the doppler where I had priaprasm, both EDVs were 0 cm/s.

I expressed my concern about having ED for the past year, but he doesn't seem too concerned about it. He assured me that a leak isn't the cause, and says it's likely that my ED is caused by a hyperadrenergic response. He put me on nightly Viagra to hopefully long-term cure the ED.

I'm willing to trust this doctor since he is well-published in his field and works at the top urologist clinic in the nation. I'm still kind of wary and depressed over the situation though since it's been going on for so long. Does anyone have any input on similar experiences? Any words are appreciated, thanks!

Anyone have any advice for me? Some days, my erections seem to be a little better, but I've been on nightly Viagra for around 90 days. I've had 3-4 "top" urologists tell me I definitely don't have a venous leak, but it's been a good year and a half since I've been able to maintain my own erection.. Any help appreciated.

if you maintain the erection only for 15 seconds it isnt very normal, sincerely it sounds like a venous leak. Dopplers are not very good at detecting this and less doctors. Do not trust doctors too much and rely even less on many urologists

I had my doubts about the doppler and brought them up to the andrologist. He assured me that my doppler results don't correlate with a venous leak. He talked about the limitations of the test with me, and while it's validity isn't 100%, there are certain indicators they look for to diagnose a leak. I also had a strong response to trimix at my first doppler. At the second doppler that I just had, only .05 cc of the medicine was injected, and a satisfactory (6/10) erection was achieved to perform the test. He said that maintaining an erection with trimix or a PDE5 inhibitor wouldn't be possible with a true venous leak.

Concerning the 15-second erections, he said my body/muscles have become conditioned to not stay hard. He explained it as a self-fulfulling prophecy. I initially had trouble with my erections and it turned into a negative feedback loop. Not breaking out of this loop won't allow me to maintain normal erections again. With Viagra, I sometimes wake up in the middle of the night rock hard, I'm guessing because my body isn't consciously following this loop.

I'm completely in the blue about this whole situation. I know it's hard to believe doctors and urologists since we're always being told it's "psychological," but they are the ones with the medical education, not us. Mayo Clinic is the #1 urology clinic in the US. He took a long time to evaluate my situation, didn't just blow me off in a 5-minute consultation.

At the beginning of these issues last year, my shaft was almost completely numb. No hot/cold, touch, pain sensations whatsoever. Now, I can feel everything except soft touch. Something's definitely improving, so maybe normal erections will come with it...

hmm i continue saying not trust many in the urologists, if they are not very specialized they will not know much more than what you can see in google and even if they are specialists in many problems they do not know to say where is the problem.

You get morning woods? Are they durable or fall when you get up? Do you get an erection without physical stimulation?

I do think many urologists slight young guys with ED by going the psychological route, but there are definitely some who give us the light of day. This andrologist didn't just tell me it was psychological. We had a good 20 minute chat about the proposed cause of my ED and the results of my doppler. Didn't just tell me the doppler looked good, he walked me through how it's read and what they look for to diagnose a venous leak. There are not many people with his credentials in the area. I had to travel 4 hours to see him, but I also saw the only certified andrologist in my state who also came to the same conclusion.

My morning wood is not consistent. Some morning I'll wake up with a decent hard-on, sometimes it's limp. When I wake up with wood, it usually goes away within a minute after standing up or after I use the bathroom. With nighttime erections, I think I've kept it up for at least 5 minutes. I can get erect without physical stimulation, but I need to be viewing something erotic (porn or even a hot text). These erections are pretty strong, but go away fast because I lose focus on the stimulation and focus on losing my erection instead.

if you have erections in the night and they last for five minutes, and you get erections without physical stimulation and you can keep them without touch while you are watching porn or whatever you havent venous leak or it is a minor venous leak or psychollogical probably

I'm surprised that none of the URO's mentioned VED?

Have you tried a ring around just the base of the shaft ... to prevent the erection from fading?
(I'm talking about the medical grade rings not a sex shop ring)

They did mention VED and rings, but I haven't been able to get either yet. Thanks for the links, I will look into them more.

New question: After I ejaculate, I notice a raised horizontal cord-like structure on the middle, top right half of my shaft. It's less than a centimeter in diamter and maybe only runs horizontally for a centimeter or so. When I touch it, it just feels like a raised ridge, not sure how to really describe it. Odd thing is I usually only notice it after masturbating. I didn't notice it at all this morning, but notice it now that I recently ejaculated. I also don't notice it when I'm hard. When I can see the "cord," there is no pain or curvature present.

Searched this issue online, and the ridge looks similar to sclerosing lymphangitis. Thought this could be the cause, but that condition seems to only happen right behind the penis head while mine is in the middle of the shaft.

I'm nervous that this is Peyronies. Can anyone offer any insight?

It's probably the following,

It happens when the lymph channels in your penis are temporarily blocked. Lymph is a clear fluid that forms part of the body’s immune system. The swelling should soon subside and will not cause any permanent problems.

I have it happen near the head of penis. It happens to me sometimes when I inject trimix. Scared the shit out of me. I thought it was a vein that got hard. The good news is it goes away after a while. Oh, masturbation seems to bring it on.
