What did the nerves control?

Postby kbota53 » Sat Oct 02, 2010 9:06 pm

Okay, I had an open RRP on July 21. Because of my gleason 9, and high volume disease, my uro could not spare the nerves, so both of them were lost. I'm trying to understand exactly what that means. What I believe is that the nerves simply controlled the valves that allow blood into AND out of the penis. So I guess that means they opened one valve and closed the other. Or maybe the nerves only closed the outlet valve which would allow the penis to fill with blood providing an erection. I'm not real clear on this.

I also understand that the viagra type drugs will not work for me because of the lost nerves. My doc has attempted two caverject injections both of which failed miserably. My Firma Medical, Ultra VED pump came in Friday, and I've tried it today for the first time. Today has been the first (semi) erection I've had since the morning of July 21. I'll continue to play with the pump while waiting on next appt for another caverject injection. If the next injection doesn't work, doc said he would order some trimix.

I read where a number of fellows who have had prostate surgery have varying degrees of success with viagra type drugs. I can only assume they had the nerve sparing surgery. Is that right?

What am I missing here? My wife and I are both very frustrated with the process so far. She goes with me to the uro doc each time, and he is showing her how to give the shots. My fear is that she will be hesitant or be bashfull about plunging the needle in, which will just hurt worse. The last uro visit, the shot was very painful. Not the caverject,...that was fine....it was the needle entering my penis that hurt.

My plumbing all worked pretty well prior to the surgery.

So, what am I missing here?

K, Dont know if I have the answer for you. I had both nerves sapred "made no difference" I can say that I have never had pain with the needle going in. Did you ask the Dr about that? I do understand the frustrations of wanting things to be back and not having success.

Hi K,

You made a comment about nerves not being the only thing affecting ED post-surgery...on another site. My ED doc told me that if the vascular (veins) haven't recovered to a good degree after surgery, no amount of nerve sparing would be relevant. Who really knows? It seems that both nerves and vascular recovery have happened for me (post trimix). I'm still ED, but oral meds are now working. I'm continually improving against my surgeons and GPs original prognosis for ED....Find a good ED Uro.

I was in what my ED Uro called a 25% group that would recover to ED oral meds only (Levitra) after trimix.

Age 54.
Open RRP in May of 2009. Gleason at biopsy, 3+3. Gleason after surgery, 3+4 with extra-prostatic extension. Stage III cancer. No incontinence issues. ED issues. Trimix for 5 months. Now, levitra alone is working at 10 mG.
