Injections & Suppositories |
What happens if the Oral meds don't work? Or what if the stuffy nose, aching muscles and weird vision are just intolerable? Not to worry - there are many more tricks in the proverbial bag. There is a whole range of treatments which involve getting drugs directly into the penis. This level of therapy is just about the only treatment for men who have nerve damage. Some of these treatments involve syringes; others call for less invasive approaches. Unlike oral meds, injections and suppositories will produce an erection even when there is no sexual stimulation. Many men who use injections wish they had started earlier. The results are usually fantastic: fast, dependable and surprisingly easy. However, knowing that there are some guys who will go nowhere near their treasured member with a needle, we'll start with the non-invasive group. MUSE Urethral Suppository |
The tablet, smaller than a grain of rice, dissolves and delivers Alprostadil directly to the penis. Alprostadil is a strong vasodilator (Makes blood vessels bigger) and causes a quick erection. It works in about 10 minutes and erection can last about one hour. |

If it works for you, it's great. How long do you need an erection anyway? After an hour, most partners have had more than enough. The drawback is that as many as 32% of men find it painful. Its use has diminished considerably with the advent of reliable oral meds, but even so, is still a first line attempt many surgeons suggest immediately after surgery. He knew that getting the Trimix formula into the penis would make for a great erection. He was also keenly aware that most men are not crazy about sticking Mr. Happy with a needle. He knew that in its normal state, the Trimix compound was not absorbed very well through the mucus membrane of the urethra. So, he altered the chemical make-up just a little bit by neutralizing the chemicals so that they would more easily be absorbed into your dick. The results seem to be impressive. Within 8 minutes, you have an erection. He also added a little lidocaine for those of us who are squeamish - it just numbed things up a bit on the inside. Heres how it works: There is a syringe with a 1.5" tube tube attached where there would normally be a needle. You insert the tube about 1 inch into your dick. Press the plunger and pull out the tube. Massage the penis. In less than 8 minutes you have an erection. The dosage is determined by starting with the smallest amount and working up from there until the best erection is produced. Dr. Shoemaker is very quick to point out that his Intraurethral Trimix is only part of his holistic approach to ED. He has a regimen of excercise to strengthen sex muscles. In 1980, Dr. Ronald Virag of France, reported that during penis surgery, he accidentally injected a patient in the wrong part of the penis with papaverine - a kind of opiate. The result was an unexpected two-hour erection. (Note to self: Cancel surgery with Dr. Virag) Can you imagine waking up from that surgery? At the same time in England, Dr. Giles Brindley had found that injecting the penis with phenoxybenzamine produced an erection within minutes. In 1996 the Japanese found that injecting Alprostadil (Prostaglandin E-1) produced very powerful erections. Don't you wonder who had so much time on their hands that they sat around and started injecting their dicks? It's a bit reminiscent of drunken fraternity boys. |

Same players, different song. Caverject Impulse is the name brand of Alprostadil - the same chemical used in MUSE. Edex is simply a different brand of the same drug. |
Caverject comes in 10 microgram and 20 microgram strengths The average dose starts at 1.25 or 2.5 micrograms, which is then increased gradually. Your doctor will adjust the dosage, particularly if it produces erections lasting longer than 1 hour. Doses should not exceed 60 micrograms. |
It is pretty much painless. There are those who insist that the shaft of the penis does not have many nerve endings. If someone says something like that, it makes you wonder if they even have one. The reality is that the needles are so fine that it is a tiny prick....uh...I mean pinch. A small price to pay for an hour of rolling in the hay. |
It works fast - producing results in 5-20 minutes. One study gave a success rate of 83%. The first injection should be given by your doctor in his office. After that, either you or your partner can inject at home. Again, there are those who insist it should become part of foreplay - those are the women you've heard about. It can be used 3 times a week but no more than once a day. |

Just sounds too good to be true? Well, now the bad news. About one third of all men find it painful. Virtually all men report an aching dick. For some guys it is excruciating. If you are one of the lucky ones who can tolerate it, it is great. If not, sex becomes painful. In that case, find that woman who thinks injections should be part of foreplay and you'll have a great time. |
Injections work. Their success rate is over 70%. Never let a doctor send you home with a prescription without having injected you first in his office. There are a lot of sloppy or rushed doctors who do not have the time, or don't want to make the time, to start with a minimal dosage in order to make sure you don't end up in the hospital. Afterall, there is the strong likelyhood that you will be sitting in his office for an hour or two waiting for the best woody of your life to go away. I always thought they should offer a quiet place for you to go try it out - but wives might object. There are horror stories about guys going it alone. Since everyone is different, the response to the same dosage can vary widely - and even the same dosage can result in wildly different responses in the same man at different times. |

Trimix and Bimix are not brand names or even terribly specific names when it comes to what is involved. As the names imply, Trimix is three drugs, Bimix is two. |
As we already know from MUSE and Caverject, the active ingredient in those treatments is Alprostadil - also known as Prostaglandin E-1 (PGE1). And if you've been paying attention at all, you now know enough to shudder at the thought of feeling pain as if your Mr. Happy were being burned at the stake. So, enter the world of drug cocktails. And like any good bartender, doctors have their favorite penis martinis as well. |

Word of Dr. Giles Skey Brindleys urological research has prompted an invitation for him to present his findings at the Annual Symposium of the Urodynamics Society. In addition, one American doubter has suggested that proof would require something beyond charts, tables and graphs. It is uncertain what that critic had in mind, but, in any case, Brindley was to prove equal to the challenge." |
Trimix was invented to reduce the amount of PGE1 which caused so many men pain. They could lower the dose of PGE1 and make up for it with papverine and phentolamine. A common trimix combination used by many Physicians is papaverine 0.5 mL (15 mg) + phentolamine 0.2 mL (0.5 mg) + PGE1 0.25 mL (10 mcg). The result is a very durable erection that lasts about two hours and has a lesser chance of causing pain. One drawback, is that Trimix is not shelf stable and must be kept refrigerated. For the quick get-away weekend of love, it requires a cooler, ice pack, and no really hot cars. But one vial can last a long time - for some guys more than 50-70 doses. That makes Trimix a really affordable choice. |
Bimix is used for the guy who can't tolerate any PGE1 at all and so the doc just uses papverine and phentolamine. The resulting erection is not quite as strong as with trimix, but if there is no pain, then Mr. Happy is really happy. |
This neat little device plunges in the needle to precisely the right depth and some models even depress the plunger to administer the drugs. You just place it on your penis and push the button. You can close your eyes and when you open them, it's all over! Well, you might want to keep an eye on what you're stabbing down there, but really, this injector makes it easy. There are prescription models and OTC models. Neither one costs very much. |
OK, so why doesn't the whole world use these therapies? Well, much of the world does. But everything in life has a trade-off. One of the side effects of papaverine is that it can cause scarring which can cause Peyronies disease. Add phentolamine and you get a greater chance of a priapism - and a free trip to the hospital to get your penis poked with more needles. So, the balancing act becomes how to get the best results with the fewest risks. And each guy has his own set of risks. Some men will battle Peyronies while other guys are in constant danger of a priapism and others want to avoid pain at all costs. Doctors experiment with different levels of each of these drugs to arrive at the best combination. The Trimix and Bimix require a compounding pharmacy (a pharmacy that mixes up its own drugs). There are usually only one or two in a state -so that can be daunting to some men - but once you get hooked up with your main pharmacist, it is really pretty easy. |

To help make injections as effective as possible, use a "pump to plump". If you pump up your penis slightly and then use an adjustable cock ring to keep it plump, the cavernosa is a much easier target. The constriction will also keep the drug in the cavernosa where it then has time to work. Many times, the drug is flushed out of the penis before it has a chance to work. Once the medication kicks in, you can remove the ring. |
